Ambulatory Academic Internist

NYC Health + Hosptials/Jacobiis a modern state-of-the-art teaching hospital located in attractive and safe residential Bronx neighborhoods located a few minutes north of Midtown Manhattan. Jacobi Medical Center is an NABPC-accredited breast cancer center of excellence and a major academic affiliate of the Albert Einstein College of Medicine Hematology-Oncology fellowship.

Responsibilities

Job summary:

The Jacobi Medical Center Academic Ambulatory Internist of the Adult Primary Care Service provides the full scope of adult ambulatory services which fall under his/her field of training including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance to adult and limited groups of adolescent patients. In addition, the Academic Ambulatory Internist, trains and supervises medical trainees and students and mid-level providers in the scope of adult ambulatory services. The scope of services includes, but is not limited to, primary care, geriatrics, medical consultation, med psych, palliative care, and selected inpatient care to support the overall missions of the Medical Center and Department of Medicine to address the needs of the population served in individualized, collaborative or population frameworks. The Academic Ambulatory Internist may be directed by the Chairman of Medicine or designee or the Ambulatory Medical Director of Operations to perform other duties in accordance with facility, corporate, funding and regulatory requirements.

Duties and responsibilities include but are not limited to:

Clinical Responsibilities:

  1. Flexible 40 hours/week including evenings and/or weekends,
  2. Off-hours coverage on a rotational basis,
  3. Medical evaluation of patients including history, physical examination, diagnostic testing, treatment plan, counseling, and referrals based on standards of care and EBM, for effective longitudinal, continuity and episodic care across the lifespan.
  4. Complete, reflective and timely documentation in the EMR.
  5. Prescription of medication and DME in accordance with New York State statutes.
  6. Review and sign off laboratory/study results with timely follow-up.

Teaching/Program Development:

  1. Develop and implement Palliative care program in the outpatient setting.
  2. Create curriculum around end of life discussion (advanced directives/healthcare proxy) for the residency program.
  3. Educate and supervise trainees at all levels and collaborate with mid-level providers through recruitment, didactic activities, precepting, and mentoring.

Quality Improvement:

  1. Participate in peer review, quality assurance, department and division meetings, and faculty development,
  2. Adherence to access, flow, productivity and quality standards.
  3. Participate in process improvement for access, quality, flow, patient experience, risk reduction, and financial viability in alignment with the goals of health and hospitals.
  4. Participate in developing/revising programs, policies and protocols for patient care aligned with patient centered medical home.

Clinical Research Expectations:

  1. Develop capabilities for clinical research and scholarly activity. Involvement in 1-2 scholarly research projects per year.
  2. Special projects as identified.

Qualifications

Requirements:

  1. Maintenance of required certifications such as infection prevention and medico legal competencies,
  2. Maintenance of affiliations with academic and professional organizations.
  3. Graduation from an accredited school of osteopathic or allopathic medicine
  4. Current New York State license
  5. Board certification or eligibility in internal medicine. If Board eligible at time of hire, must be board-certified within 3 years of hire date.
  6. Current DEA registration
  7. CPR certification
